Title of the Project: Internet-Based Teacher Training Resources for Working With Paraprofessionals

This is a research project examining the effectiveness of providing Web-based training to teachers and future teachers, entitled 'Supervisors of Paraeducators Self Study Program'. You are being invited to participate because you are in a class or training program that is using these instructional materials. You must be 19 years of age or older to participate.

You are not being asked to complete additional activities as part of the research and it will require no additional time on your part. You are being asked to allow us to include data from your responses to the activities in the Web-based training in a group summary and analysis. The purpose of this analysis is to judge the effectiveness of the training materials and provide the researchers information on improving the program. Your participation in this study will benefit teacher education programs and teachers in general. The information gained from this study may help us to understand how to make Web-based training programs for teachers more effective. There are no known risks or discomforts associated with this research.

Any information obtained during this study that could identify you will be kept strictly confidential. Student performance and evaluation data is maintained electronically in a database associated with the Web site hosting the course. Access to this information is restricted to course instructors and the researchers. Course instructors may view individual student performance data as a regular part of teaching the course. Evaluation data is presented to participating instructors as group data and individual student names are not associated with comments. This follows typical procedures for course grading and evaluation.

The results of the summary and analysis may be presented at professional meetings and in the professional literature. Results of the data collection will not be reported in a way that individual participants can be identified. Quantitative data will be reported by group summary only. Narrative data will be reported without attribution to individuals.

If you choose not to participate you will still participate in all training activities but we will not include your data in the summary and analysis. You are free to decide at any time not to participate without adversely affecting your relationship with the investigators, the University of Nebraska or your participating school. If you wish to withdraw your permission you may do so by contacting one of the principal investigators. If you choose to withdraw at any time, none of the prior data collected from you will be included in the published summary and analysis.

You are voluntarily making a decision whether or not to participate in this research study. You are asked below to indicate that you are at least 19 years of age and that you agree to participate. You may keep a copy of this letter for your records by printing this document from you browser prior to submitting the form. If you wish to obtain a copy at a later date you may do so by contacting one of the principal investigators. Your submission of this form will indicate your consent to participate.

You may ask any questions concerning this research and have those questions answered before agreeing to participate in the study. You may telephone or email the investigators at any time at the telephone numbers and email addresses below. If you have any questions concerning your rights as a research subject that have not been answered by the investigators, you can contact the University of Nebraska-Lincoln Institutional Review Board telephone (402) 472-6965.
